Unfortunately, we do not accept insurance at this time. Insurance companies vary on coverage for ketamine treatments, and you are welcome to apply for reimbursement; we will provide you the necessary documents to do so. Our experience shows that about 30-35% of patients seeking treatments for mental health are receiving some reimbursement. The rate for reimbursement rate for pain treatments is generally much higher. We remain hopeful that insurance companies will one day join the legions of others who recognize the effectiveness of ketamine and begin providing better coverage for patients.
In addition, through our partner app Osmind, we can generate a superbill that you can submit to your insurance company using the Osmind app. You may receive costs back, or it might be applied to your deductible. This superbill can be added into our app with your insurance info, and can be submitted for you. Many patients struggle with self submitting to their insurance companies; our partner app Osmind makes the process quick and easy.
Unfortunately, our cost in providing ketamine infusions is not covered by any insurance companies to the extent necessary to meet expenses.

We do not require a prescription or referral from a psychiatrist or physician. We do need to speak to with another clinician or organization who has managed the condition which you are seeking treatment. This is to confirm diagnosis, to learn more about you, and personalize a treatment plan for you. The clinician can include a psychiatrist, psychologist, therapist, general practitioner, or pain specialist.
If you do not have a current clinician, we can refer you to get an assessment to help ensure ketamine treatment is a good fit for your condition.
Dr. Vanlierop will also evaluate your prior medical history before your ketamine treatment as well.
Payment is required either before, or at the time of all services. Arizona Ketamine Specialists accepts payment for your treatments via cash, money order, credit card, Flexible Savings Account (FSA), or a Health Savings Account (HSA).
